NTSB Releases Report on 2021 Houston Shoving-Movement Accident

The National Transportation Safety Board (NTSB) has issued a report and three safety recommendations on shoving movements following its investigation of the Oct. 29, 2021, accident involving a Watco Dock and Rail LLC (WDRL) conductor killed when a train collided with a combination vehicle at a grade crossing in Houston, Tex.

NTSB Issues Preliminary Report on CSX Conductor Trainee Fatality

The National Transportation Safety Board (NTSB) has released its preliminary report for the investigation of the Aug. 6 accident involving a CSX conductor trainee fatally injured during a shoving movement at the CSX railroad yard in Cumberland, Md. The agency said future activity will focus on CSX employee training and close clearance identification practices of the standing locomotives.

NTSB: Brakes on Trolley Involved in SEPTA Derailment Disabled to Allow for Repairs  

A preliminary report on the derailment of a Southeastern Pennsylvania Transportation Authority (SEPTA) trolley issued on Aug. 21 by the National Transportation Safety Board (NTSB) has revealed that the brakes on trolley 9107, which, on July 27, derailed and struck an SUV and a restaurant, had been disabled on the day of the accident to allow for air compressor replacement.
